The Life will be recognizing Cancer Survivors from ALL cancers during the month of October culminating in a presentation.

For the Cancer Survivors we are requesting the following via upload to our website no later than Sunday, October 2:

1. First & Last Name
2. Type of Cancer
3. Amount of Time Cancer Free
4. Upload a Photo
5. IN ADDITION TO PHOTO Upload a 1-3 minute Video (optional ) covering the following:

-Name
-Type of Cancer
-How Long Cancer Free
-Brief Overview of Your Healing Journey
